Case Management
An alternative to sending patients home before they are ready for a home environment, Case Management works to provide nursing care with an emphasis on rehabilitation and works with the patient to prepare them for daily activities and living. This home-care setting in the hospital can assist with a smooth transition from the hospital to the home.

Criteria
- Must have been hospitalized in an acute care setting for at least 3 days in the past 30 days.
- Must be ordered by a physician.
- Patient must be medically stable.
- A realistic discharge plan is in place.
- Rehabilitation services intervention needs to be medically reasonable and necessary.
Insurance Coverage
Medicare and Medicaid cover the service for a specific length of stay. Some other insurances may cover the service, but prior authorization is necessary.
